mediatek: add EEPROM data for BPi-R64 2.4GHz wmac

EEPROM data extracted from vendor image found at
http://forum.banana-pi.org/t/bpi-r64-mt7622-mac80211-wifi-driver/10246/77

MAC address in the EEPROM has been zero'd which results in random
address on boot.
